Please submit the signed document (any staff member may be used as your witness) along with the form requesting information about your bank. As mentioned in our last interview, your pay will be automatically deposited into your account every other Thursday.

Confidential Information Agreement
As an employee of Financial Bonus Inc. I ___ hereby agree and acknowledge the following:
l. That during my employment certain confidential information may be disclosed to me, including customer and business details
2. That at no time during or after my employment I will offer any of this information to others, including other Financial Bonus inc. staff members. (Note: Never recycle company files before putting them through the shredder.)
3) That I will not make copies of any documents marked "confidential" without the written consent of my supervisor.
4) That I will personally return any Financial Bonus Inc. business documents marked "confidential" to reception when I leave the company so that they may be destroyed.
